  • How do I add a form to my website?

  • How do I access the submissions?

    Canoply will notify you whenever a form is filled out with a link to the new submission. Just follow the link and download the the form as a PDF, CSV or text file. You control how you receive these notifications and can view all of the forms in one place.


  • Are there any setup costs?

  • How is my patient's data stored?
